Silver and Platinum include 24/7 virtual urgent care plus ongoing virtual primary care — both at no copay. Connect with a licensed clinician by phone or video for common health needs without leaving home.

Good for: cold/flu symptoms, sore throat, skin issues, follow-ups, medication check-ins, and guidance on whether you need urgent or in-person care.

Not for emergencies. Chest pain, severe bleeding, or stroke symptoms — call 911.

Gold and Platinum use simple, flat amounts for in-person care through the nationwide MultiPlan PPO network. The amounts shown are what you pay at the visit.

$25 for primary care and specialist visits. $50 for urgent care, labs, and basic X-rays. $0 for ACA-required preventive visits and immunizations.

Advanced imaging (CT, PET, MRI) and certain hospital outpatient services can fall outside this structure. If you’re unsure, ask support before scheduling.

Gold and Platinum include all ACA-required preventive services at $0 — annual wellness visits, recommended immunizations, age-appropriate screenings, and pregnancy-related preventive care.
